Official USBC league application for adult, youth and adult/youth leagues. Complete league, center, officer, schedule and league-funds information.This application was included in your league packets, however, it is available for download in case you made a mistake, or damaged the original copy.

Adult Membership Application

Use for bowlers joining or renewing membership through your certified adult league. This is the form that is included in your season beginning league packets, but a downloadable version. Secretaries using BLS software, this application is in your software as well. If you want to apply online? NO Problem! Presides at meetings, appoints committees as required, verifies league accounts and helps ensure the league follows its adopted rules and USBC requirements.
